Surgeon Simulator: Experience Reality is a VR-only standalone title developed and published by Bossa Studios, bringing the infamous, deliberately awkward surgery simulation gameplay of the 2013 original Surgeon Simulator 2013 into virtual reality for HTC Vive, Oculus Rift, and PlayStation VR. Rather than a direct port, the game was rebuilt from the ground up for VR, replacing the original's mouse-and-keyboard "fumbling" controls with motion-controller input that lets players use both hands independently to grab, poke, slash, and yank surgical tools and organs in a physics-driven sandbox.

The core experience consists of five transplant procedures—heart, kidney, brain, eye, and teeth—each performed across three increasingly chaotic environments: a standard hospital operating theatre, the back of a moving ambulance, and zero-gravity outer space, for a total of fifteen levels. VR-exclusive additions include a handheld camera to inspect your surgical work from any angle, brain surgery performed in complete darkness, and new achievements tailored to the VR experience. While the original game's cult reputation stemmed from the gap between player intention and execution via awkward mouse controls, reviewers noted that the VR version's comedy now comes from the physical challenge of manipulating objects with motion controllers, though some criticized the control scheme and tracking as frustratingly obtuse. The title launched at $19.99 USD across all platforms, with a free teaser demo, Surgeon Simulator VR: Meet The Medic, previously released in April 2016 as an HTC Vive exclusive featuring Team Fortress 2's Medic operating on the Heavy.
