You play as Justine, a rookie federal marshal whose mind is shared with Dr. Carnival, a deranged killer. Together you must eliminate every serial killer trapped in a strange hotel on the border between Pennsylvania and West Virginia, then survive long enough to escape.

The game mixes side-on movement with depth, so fights need positioning, timing, and clever use of abilities. When you die, a Slasher Phantom is born that repeats your previous run's movements and can damage enemies, turning failure into an extra ally.

Staying aggressive covers you in blood and fills the Skull Gauge, which unlocks Carnival Awakening, a powerful special attack. The hotel is split into seven zones inspired by different horror sub-genres, each ending in a boss fight. A free Under New Management update reworked movement, combat, and multiplayer balance, and Steam lists online co-op and PvP features.
